Residential Treatment Programs

Persons Served

Designed to support adults (18 years and older) with any or all of the following diagnoses: Severe and Persistent Mental Illness (SPMI); Intellectual/Developmental Disability (I/DD); Autism Spectrum Disorders (ASD); Co-occurring Substance Use Disorders; Personality Disorders and/or a history of Traumatic Brain Injury (TBI).

Some individuals served additionally may present as behaviorally complex and/or medically complex.

Residential Treatment Programs are capable of supporting individuals who have been adjudicated and found Not Guilty by Reason of Insanity (NGRI), Incompetent to Stand Trial (IST) or those who are court ordered to a behavioral health rehabilitation program such as those on an Assisted Outpatient Treatment (AOT) Order.

Treatment Scope

Residents in Turning Leaf’s Residential Treatment Programs live in Adult Foster Care (AFC) homes licensed by the State of Michigan; specialized in serving adults with mental illness and developmental disabilities. All programs have 24 hour awake staffing trained to assist residents with unique and challenging behavioral needs. Staff are trained to prompt, guide, assist, support and observe and monitor meal preparation, laundry services, routine household care, activities of daily living (ADLs), shopping, money management, socialization/relationship building and monitoring, supervision and protection. Within the Residential Treatment programs, residents are encouraged to provide and receive peer support, receive assistance or encouragement with activities of daily living as necessary according to their individualized plan of service and participate with health and case coordination services to improve their overall wellbeing.

Residential Treatment programs are inclusive of all transportation, Community Integration, Skill Building, Coordination of Primary and Behavioral Healthcare, on-site work programming and crisis intervention services. Milieu may vary based on location and is dependent on the individuals served at the given time.

Depending on location and program, Turning Leaf residents will have access to a combination of Turning Leaf employed and/or county-based, Community Mental Health treatment providers including, but not limited to: Psychiatrist, Nurse Practitioner, Primary Care Physician (PCP), Licensed Masters Level Social Worker, Case Manager/Supports Coordinator, Community Integration Coordinator, Health Coordination Services and Behavioral Psychologist.

Home and Community Based Services (HCBS) Rule Statement

Turning Leaf is committed to establishing and maintaining compliance with the requirements for the Home and Community Based Services (HCBS) Rule throughout all our programs. We understand the intent of the rule is to provide consumers with the opportunity to have Choice in the services they receive. To that end, we believe in privacy, dignity, full participation in life’s decisions and community inclusion. Any exceptions or restrictions to the specific requirements of the HCBS Rule will be documented within a behavioral treatment plan with necessary data collected and a plan in place to reintegrate into the full inclusion of the home and greater community.
